Fill in this card either electronically, by completing the text fields on the pdf file available from this page, or manually by printing it out. The form is 8.5" x 11" and folds on the dotted line. Fold the form back so the print areas face out. Use tape or glue to affix the two ends of the sheet together and you now have a complete Pledge Card. Turn it in to the "Wear It California!" Campaign Boat Team and you may receive a free life jacket!
